Support & Wellness Advisory Service
The Support and Wellness Advisory Service will:
- work to ensure the patient’s best interests are considered at all times whilst maintaining their dignity and quality of life.
- operate to reduce the disadvantage and effects upon the people.
- quality assure the delivery of advice to ensure the highest standard is consistently applied.
- assist people in understanding and navigating the rehabilitation process, improving their experience whilst maximising independence.
- facilitate the creation of a “life-bubble” for the patient with the specific aim of minimising the emotional, psychological and social issues in the patient’s personal life during the treatment/recovery/rehabilitation phase whilst maximising the opportunities for the patient to enjoy as independent life, purposeful living and social interaction as possible.
- help patients say what they want, inform them of their rights, assist them with issues including (but not exclusively) benefits, finances (debt), housing and other non-medical matters, supporting them as appropriate.
- provide appropriate information on statutory benefits, in accordance with current guidance from the Secretary of State, so that patients, families and carers are informed about their entitlements and enabled to claim and receive benefits.
- provide advice on the implications of subsequent changes to patients’ circumstances and support/signposting to appropriate services if required.
- promote individual resilience and self-help where this is appropriate.
- work to complement medical services.
- provide a bridge to social and functional support to patients and their families/carers.
- act as a continuing contact for patients and their families/carers.

The Support & Wellness Advisory Service is a patient advisory body for the Cheshire & Merseyside Acute Rehabilitation Network NHS.
We provide free, independent, accessible and confidential information, advice and support to patients, their careers and families on non-clinical issues.
The Service has been introduced to help ensure that individuals do not suffer through ignorance of their rights or of the services available, or through an inability to express their own needs.

Benefits
We provide essential information and advice for patients and/or those caring for someone who is ill or severely disabled. We can assist you in completing benefits forms, and write letters and make phone calls on your behalf.
Appointeeship
For those incapable of managing their own finances, unable claiming benefits, paying bills or managing money due to an accident or illness they may need an appointee to provide help. We can advise you on this if needed.

At Acute Need, we can help with many other subjects as well;; however, if we cannot assist with you we will help you find another organisation that can.
You can access a Support & Wellness Advisory Service Advisor through…. A Keyworker or other NHS staff member who will refer you to one of our Advisors or you can contact us directly via our office number, Email, or through drop-in enquiries, surgeries and planned appointment sessions. We can also provide home visits (resources permitting & where appropriate)
